Job Description:
A company is looking for a Medical Director - Post-Acute Care
Management - Care Transitions.
Key Responsibilities
Provide daily utilization oversight and communication with network
physicians and hospitals
Conduct peer-to-peer conversations for clinical case reviews and
respond to clinical inquiries
Collaborate with leadership to develop action plans and monitor
progress towards goals
Required Qualifications
Board certification as an MD, DO, or MBBS with a current
unrestricted medical license
Ability to obtain licensure in multiple states
3+ years of post-residency patient care experience, preferably in
inpatient or post-acute settings
